This injury is usually the consequence of self trauma (scratching or shaking of the head) or is caused by inflammatory lesions involving the ear, such as poorly treated, neglected or recurrent ear infections. In fact, in the case of itchy or painful ear infections, our animal friend will tend to shake his head strongly, to shake through gods movements that can lead to extravasation of blood and lesion formation.

Technically theotohematoma is an accumulation of blood inside the auricle and has an evolution that is defined as dynamic over time. In fact, at first it appears as one fluid-filled, fluctuating and fluffy formation, which, however, over time can thicken up to the point of weighting and deforming the ear. The lesion that is generated will have a particular conformation and will in fact cause the formation of the so-called «cauliflower contracture».

The stasis of the blood at the level of the cartilage derives from the fact that thedog ear he does not have much subcutis and therefore the blood has difficulty in reabsorbing and if it does, it does so very slowly. Therefore, there isn’t one particular auricular conformation that predisposes to this kind of injury; all dogs can be affected, but it has been found that some breeds, such as those with long and drooping ears, are more predisposed than others.

Noticing the presence of an otohematoma is not difficult. In fact, we will be able to view a swelling in the ear of our friend. It will be a warm, sometimes painful swelling of varying size. It will be of fundamental importance to go to the veterinarian when you notice a new and rapidly evolving formation, to understand if it is an otohematoma or instead of a neoformation of another nature or a cyst. It is always of fundamental importance in fact observe our friend daily to try to read any variation in behavior: any alteration of our friend’s normal behavior always has a reason and is never accidental. Understanding his signals will be essential to help him and spare him unnecessary suffering.

Once at the doctor, him will carry out a thorough clinical examination, to view the injury, and develop a timely medical history to try to reconstruct the cause of that injury, and therefore the reason that generated it. In particular he will want to know if there have been any previous ear infections or never healed, allergic ear infections that may have given rise to such a formation.

Once a correct and complete diagnosis has been made, the doctor will choose the best therapeutic intervention for the specific circumstance. Each therapeutic option is based on: the identification and resolution of the primary cause of otitis and the prevention of relapses.

There medical therapy therefore consists in administering drugs such as antibiotics, fibrinolytics and anti-inflammatory drugs and eliminate the root cause. Once the triggering cause has been identified and resolved, it will be possible to operate directly on the lesion.

You can opt for three different types of resolutions depending on the specific case and the given condition.

In first place, when the situation permits, you can try to allow the otohematoma to regress on its own, therefore waiting for the blood accumulated in the pavilion to reabsorb itself. This is an eventuality that could be used only in some specific cases, such as in the event that there is no bacterial infection: however, it must be considered that it is a resolution that takes a long time and that has a great side effect. , represented by the fact that, following the reabsorption of the blood, the ear shrivels up losing its physiological shape. The result will therefore be bad from an aesthetic point of view.

As an alternative to this solution, there is the possibility of drain the otohematoma (to allow the removal of all the blood accumulated inside) and perform a compression bandage. This eventuality finds application above all in cases in which there are recent lesions and the hematoma has not yet organized.

In fact, in the case of extensive or chronic hematomas, they are to be preferred surgical techniques that allow to eliminate the hematoma, drain the lesion but at the same time allow the prevention of relapses. We will also try to preserve the normal appearance of the ear, avoiding thickening and exuberant scars, in order to solve the problem while ensuring the best possible aesthetic result.

These surgical therapies consist of performing one Italic S incision along the entire hematoma, so as to allow the escape of accumulated fibrin. We will therefore proceed with complete drainage of the lesion. In this case it will be necessary, to prevent relapses, to create and guarantee an optimal apposition of the tissues by means of sutures, applied parallel to the vascular structures. A light bandage will then be necessary to immobilize the ear and ensure optimal sealing of the points. Also in this case some degree of ear curling should be feared, but clearly it must be considered that the aesthetic result will be much better the earlier the intervention is. In all cases in which surgery is used and therefore the affixing of stitches, it will be appropriate, under the advice of the doctor, to use the elisabetta collar, which will avoid scratching and self-trauma by our friend.

Here is how, even in this case, our role, ours, will be fundamental attention to the life and health of our friend. A routine veterinary medical examination, a periodic check of the state of health of the ears and an adequate and timely cleaning of the ears, can in fact be an excellent way to prevent ear diseases for our friend. However, if all this is not enough, it will be important to intervene promptly. In fact, in the case of suspicion of a simple otitis, or of any ear alteration, it is necessary immediately go to the vet to try to correct the problem, resolve the situation promptly and avoid unnecessary suffering to our friend.

In any case, what we do for him will always be too little, compared to the infinite love that he will give us every day.
